The conference will feature national leaders in Care Coordination demonstration and evaluation projects and highlight models of inter-professional education that are being implemented across the United States. The morning panel will focus on current models of inter-professional care coordination for a cross-section of populations and the challenges of implementation. The afternoon panel will focus on models to prepare health care professionals for inter-professional and team-based practice and the implications for educational and training programs.
