6:00pm - An Overview of Issues Impacting Care Coordination and the Social Work Role As a Result of the Affordable Care Act (ACA)
Emma Barker, MFA, LMSW
The importance of the Affordable Care Act in promoting links between medical services and long term care services. Innovative models being promoted, such as the Patient Centered Medical Home and Accountable Care Organizations. Social work knowledge and skills in ensuring beneficiaries receive appropriate services that link health and medical needs with long term care. Challenges in the provision of care coordination.

6:30pm - The Role of the Social Worker in Providing Case Management/Client Care Coordination Services in a Changing Health Care System   
Amandia Speakes-Lewis, PhD, LCSW-R
The Bridge Model of Transitional Care as a method to enhance patient discharge to home. Strategies for providing services within a changing healthcare system. Effective case management strategies for working with diverse populations. Options to effectively meet the challenges faced in providing case management/care coordination. Evolving roles of the social worker as a mechanism of effectively caring for clients.

7:30pm - Case Managing Culturally Diverse Clients in a Changing Health Care System
Louis A. Burns, MSW, CASAC
The importance of delivering quality case management services in a changing health care system or model. Understanding the challenges faced in case managing culturally diverse clients. The importance of conducting a culturally, including spirituality and faith, appropriate client assessment. Assist clients in developing strength based client-centered care and treatment plans, including the utilization of the SNAP and SMART models and developing strength-based client-centered discharge plans.

9:00pm - Participants submit evaluations

Accreditation
Social Workers: This course has been approved by NASW-New York State for three (3) contact hours.
CASACs: An application has been submitted to the New York State Office of Alcoholism and Substance Abuse Services for three (3) clock hours towards initial credentialing and renewal.
All other healthcare professionals will receive certificates of attendance.
Please check with your accrediting board with respect to receiving credit.
