Care coordination is one of the major cornerstones of health reform, the way to improve care while containing costs. Nowhere is it perhaps more challenging and important than in the delivery of care to high-need, high-cost patients. The experts we have assembled for this program have all wrestled with the challenges of implementing care coordination for special high-need populations. They will share their experience and insights on what works and what doesn’t as well as what we should watch for and expect as we continue to roll out health homes and other new forms of care delivery.
