Formed in 2008 as a partnership between SWLI and the American Society on Aging (ASA), the National Coalition on Care Coordination (N3C) is a national membership coalition of organizations dedicated to improving the quality of care for individuals through support for care coordination in health and social sectors.
The Coalition is composed of 114 individual members representing 37 organizations, including consumer, aging, social service, health care, family caregiver, and professional organizations.
The N3C is staffed by the Social Work Leadership Institute at The New York Academy of Medicine with the support of The Atlantic Philanthropies.
Purpose
The purpose of the National Coalition on Care Coordination (N3C) is to advocate for enactment of public policies that support care coordination.
The coalition's goal is to effectively link health and long-term care on behalf of vulnerable populations – with particular focus on older adults.
The coalition believes that care coordination should:
- be patient centered;
- be supportive of family and informal caregivers;
- be accessible;
- take an interdisciplinary approach;
- focus on chronic care and health care transitions;
- bridge health and social services;
- employ a comprehensive assessment; and
- implement and monitor a flexible care plan.
