Transferring Stock to The New York Academy of Medicine

Instructions for a wire transfer of stock from a brokerage account to NYAM:

1. Draft a letter of authorization to your broker with the following language:
   
   This letter represents authorization for transfer of [# of shares] shares of [name of stock] from [name of donor] to The New York Academy of Medicine. This gift is to be applied by NYAM as follows:

   ---

   Please forward an acknowledgment of this gift to [your name],[your address, city, state, zip].
   
   Please provide a copy of this letter Charles Schwab and to NYAM.


2. Have your broker send the wire transfer instructions, with a copy of your letter of authorization, to Charles Schwab:

Instructions for a transfer of stock in the form of physical securities to NYAM

1. Send the stock certificates by certified mail directly to the NYAM contact below. Do not endorse, sign, date or insert the name of The New York Academy of Medicine as transferee on the certificates. Leave the certificates blank when you mail them. NYAM contact:

2. Stock powers, if required, should be sent under separate cover to the NYAM contact below. Fill in only the name of the security, the number of shares, certificate number(s), the date and your signature exactly as your name appears on the stock certificate. You do not need to have your signature guaranteed.


3. Draft a letter of authorization to NYAM with a copy to our broker (contact information below) with the following language:
   
   This letter represents authorization for transfer of [# of shares] shares of [name of stock] from [name of donor] to The New York Academy of Medicine.
   
   This gift is to be applied by NYAM as follows:

   ---

   Please forward an acknowledgment of this gift to [your name],[your address, city, state, zip].
