Many older adults prefer to remain at home rather than moving to nursing homes or other institutional settings. However, the frail elderly and others with multiple medical conditions may need supportive services to remain in their homes. With funding from Atlantic Philanthropies, NYAM's Social Work Leadership Institute (SWLI) has been examining and advocating for appropriate care coordination services for older adults. To ensure that the perspectives of older adults are incorporated into their work, SWLI contracted with the Center for Evaluation and Applied Research to conduct and analyze focus groups and qualitative interviews with diverse older adults living in New York. Findings are described in our report: "Perspectives on Care Coordination: Voices of Older New Yorkers."
With funding from the John A. Hartford Foundation, the Hartford Partnership Program for Aging Education (HPPAE) works to: 1) meet the workforce demand for geriatric social workers by training and educating masters level social work students in older adult care and 2) establish a specialized aging practicum model that can be replicated in social work programs across the country. A project of NYAM's Social Work Leadership Institute (SWLI), HPPAE promotes the development of innovative partnerships between universities and community-based agencies and provides students with wide-ranging, hands-on experience in older adult care. The Center for Evaluation and Applied Research, working with the Department of Social Welfare at the UCLA School of Public Affairs, is implementing an evaluation of HPPAE, which measures both process and outcomes at the institutional and at the student level. Evaluation reports include: "Implementation of the Hartford Partnership Program for Aging Education: Lessons from the Field, February 2008," and "The Hartford Partnership Program for Aging Education (HPPAE) Cycles 1 & 2, 2005-2009 Summary Report."
